By signing below, I acknowledge that I have read, understand, and agree to all registration, financial, fundraising, media, attendance, and commitment polices outlined in this agreement.
I understand that my player's roster position represents a commitment to the team and organization. I agree to support my athlete's participation and understand that all fees paid to the organization including the $350 registration fee, are non-refundable except at the sole discretion of the organization.
I acknowledge the fundraising expectations and understand that participation helps support the continued success and affordability of the program.
I also acknowledge the player image and media release policy and have selected my preference above.
